Emergency Room Diversion
In our community, one of the problems is that many of our people end up going to the emergency room, even when it's not necessary - or when it could have been prevented. We are working to diminish the frequency that we go to emergency, offering options so that you can take better care of your family. Save time and money, and keep your family healthy. Our Community Health Workers (CHW) teach the importance of having a primary care doctor or a medical home, in order to save time and money by avoiding trips to the emergency room.
Take control of your family’s health now — start preventing illnesses and expenses.
Plan ahead! Don’t wait until your family gets sick.
1. Keep up with regular check-ups and exams, especially if you or one of your family members already have an illness such as asthma, diabetes, cancer, heart disease, high blood pressure, or high cholesterol.
2. Follow your doctor’s or nurse’s advice about eating healthy, maintaining a regular exercise routine, and taking any prescribed medications.
3. If you don’t have health insurance, make an appointment to see if you qualify for the Medical Access Program (MAP), Medicaid, or CHIP. The process can take a while, but it’s easier to navigate the system when you’re feeling well and have professional help.
